The following information to complete
the application:
- Federal Employer Identification Number/Federal Tax Identification Number
- Average annual volume of work performed over the past 5 years
- A copy of your last three years of OSHA 300/200 logs
- References from at least three major suppliers
- EMR Rates for last three years (for companies with 10 or more employees)
- Surety broker information including bonding capacity per job, aggregate, date of last bond and surety rate
